Marvel superhero Mark Ruffalo plays two different characters in the teaser for the upcoming HBO miniseries “I Know This Much Is True,” but they’re a far cry from the Hulk and Dr. Bruce Banner—the actor plays Dominick and Thomas, two middle-aged twin brothers, at least one of whom is struggling with his mental health.

Adapted from Wally Lamb’s bestselling novel of the same name about betrayal, sacrifice, and forgiveness, the six-part family drama also stars Stern Show veterans Rosie O’Donnell and Juliette Lewis as a social worker and a graduate student, respectively.

“Your brother is seriously mentally ill,” Rosie’s character tells one of the twins (above).

The teaser isn’t always clear about which character Ruffalo is playing and when, but one thing is for certain: the twins’ relationship is on the rocks. “I need you to stand up for me for once,” one brother tells the other at one point.

From Derek Cianfrance (“The Place Beyond the Pines”) and executive produced by Ruffalo, among others, “I Know This Much Is True” also stars Melissa Leo, Archie Panjabi, Imogen Poots, Bruce Greenwood, and Kathryn Hahn.

“I Know This Much Is True” premieres April 27 on HBO.
